Our team culture is built around collaboration, respect, and continuous learning, creating an atmosphere in which both staff and patients can flourish.Key ResponsibilitiesConduct thorough initial evaluations of patients to identify their physical capabilities, treatment goals, and specific needs.Create and implement individualized treatment plans that are designed to enhance patients' strength, mobility, and functional independence.Utilize evidence-based therapeutic modalities, including therapeutic exercises, manual therapy, and advanced rehabilitation techniques, to address physical impairments.Regularly monitor and document patient progress, making necessary adjustments to treatment plans based on their evolving needs and advancements.Collaborate effectively with an interdisciplinary team, including physicians, nurses, and occupational therapists, to ensure holistic and coordinated care for each patient.Provide education and resources for patients and their families on effective home exercise programs and strategies for maintaining optimal physical health post-treatment.Stay updated with the latest industry practices and ensure compliance with all state and federal regulations pertaining to physical therapy.Patient Population / Client FocusThe ideal candidate will interact with a diverse patient population, including individuals recovering from surgeries, sports injuries, or those managing chronic conditions. Our focus includes both the active adult demographic and the geriatric population, with a particular emphasis on functional mobility and athletic rehabilitation. You will utilize tailored approaches to address the unique needs and goals of each patient, helping them achieve their rehabilitation objectives in a supportive environment.QualificationsMust possess a valid licensure as a Physical Therapist in New York State.A Bachelor's Degree from an accredited institution in Physical Therapy is required; a Master's Degree or Doctorate is preferred.Excellent communication, organizational, and interpersonal skills are vital for engaging with patients, families, and the healthcare team.A strong ability to work collaboratively within a team-centric environment and dedicate oneself to professional development is essential.Preferred ExperienceWhile a minimum of two years of experience in a physical therapy role is required, candidates with specialized experience in a skilled nursing facility or with the geriatric population are highly desirable.
