The role involves delivering outpatient rehabilitation therapy in patients' homes, maintaining documentation, and collaborating with interdisciplinary teams to support patient recovery. Certifications: BCLS required. Education Requirements: Not specified. Years of Experience: Minimum of 1 year of skilled home health experience required. Location Requirements: Candidate must be within a 50-75 mile radius of the area. Local candidates are accepted. Schedule: 8-hour days, Monday through Friday, with a minimum of 40 hours per week. Additional Information: Candidate is expected to have their own transportation. The system groups clients to minimize drive time, with occasional maximum radius of roughly 60 miles (60-90 minute drive time) per day. Candidates should be prepared to work in a home health setting, utilizing charting systems such as EPIC and Rover. Submittal Requirements: Candidates must answer whether they have prior experience at a Providence facility (location and dates), and provide two references covering the last 1 year during the compliance process and prior to start.
