Description: Health items need to be collected within 48 hours. Must use the facility timeclock. If a badge is lost, the clinician will have to pay to get it replaced.

Type of facility: Long-term care and skilled nursing facility.

Total beds: 300 divided into 6 units.

Ratios: RN Supervisors are not assigned to resident cares/tasks. RN ratios will vary per unit based on the unit size. The majority of floor nurses are LPNs.

During weekdays, there will be 3 Nurse Managers assigned to 2 units. On weekends, 1 Nurse Manager will oversee all units. In addition to the Nurse Manager, there will be 2 additional nurses on each unit.

Evening staffing will include 1 RN Supervisor and floor nurses (either LPN or RN). Nights will have 1 RN Supervisor as staffing allows and 1 floor nurse (RN or LPN) per unit.

The facility accepts long-term care, skilled, rehabilitation, and dementia residents aged 18 and older. Hospice care is accepted in-house, with dedicated hospice rooms available.

Responsibilities include running the building during evenings, nights, and weekends. The RN Supervisor oversees floor RNs, LPNs, and CNAs. The RN Supervisor is not assigned to resident cares/tasks but serves as a resource for others. Responsibilities include monitoring changes in residents' conditions and scheduling outside services.

RN-only tasks include central line cares, chest tube care/management, and IV starts. There are no tele monitors, and RN staff do not read strips. RN participation in care plan meetings is optional. Physician availability after hours is through a House MD and nurse practitioner by phone.

The interdisciplinary team includes PT, OT, SLP, case managers, social workers, and dieticians. Respiratory therapy staff is not on-site 24/7. Medications are administered from pill bottles, with access to an emergency drug closet when needed.

Shifts do not allow for self-scheduling, and weekend requirements are every other weekend. Holiday work is required if it falls on the scheduled day. There are no call or standby requirements. This position does not include floating. Dress code allows any color of clean, professional scrubs except camo. Unit-specific orientation is 3 days.
