• Monday-Friday 12AM-8AM
• Tuesday-Thursday 4PM-12AM, Saturday & Sunday 12AM-8AM

Summary : In this role, you will be responsible for coordinating Mental Health services for adults residing in a 24-month transitional OMH licensed Community Residence. The overall job summary includes assessing resident's needs and developing a counseling plan to ensure that the resident is linked to needed services both inside the residence and within the outside community. Responsibilities:

• Assist with coordination and implementation of clinical activities that provide culturally competent service planning to help residents with identifying needs according to the mission of the program.
• Assist the Assistant Program Director in the admission process and intake interview for screening prospective residents.
• Assist Assistant Program Director in planning and organizing operations in accordance with all the overall objectives of NYS OMH 595 as well as to ensure compliance with both NYS OMH and Medicaid reimbursable procedures.
• Assist with the discharge of residents from the program.
• Participate in incident/complaint review.
• Provide assistance to mental health counselors with emphasizing psycho-education, medication management, money management, entitlements, securing permanent housing and activities of daily living.
• Maintain resident-based caseload as determined by the needs of the program.
• Complete weekly progress notes in accordance with Medicaid billing procedures.
• Meet with assigned residents on a timely basis to establish a counseling relationship.
• Document all scheduled and impromptu interactions with assigned caseload and other residents in a manner consistent with OMH 595 guidelines and Medicaid.
• Coordinate crisis intervention with mental health and/or substance abuse providers such as hospitals, detoxification clinics, day treatment programs, therapeutic communities, etc.
• Participate with Assistant Program Director in designating caseload assignments to mental health counselors for case management services. Assist in outreach activities and maintain working relationships with institutions and community- based organizations (CBOs).
• Interact and engage residents to facilitate positive relationships to assist them to move toward a positive clinical outcome.
• Conduct Community Meeting and other groups as required.
• Report to Assistant Program Director all deficiencies within the facility to assure all safety standards are met in accordance with program policy and mandates. Complete the six month Maintenance Log and schedule Fire/Safety drills, educating all staff and residents about these programs.
• Supervise and support other staff as needed.
• Be available for 24 hour program needs.

Requirements:

• Licensed such as LMSW, LMHC, or LMHC-LP with experience and knowledge of the mental health/ Substance
• An associate's degree in a human services related field AND Three years' experience in human services; OR a high school diploma AND five years' experience in human services.
• Training in conflict resolution and solid clinical skills are a plus. 
• Experience working with mentally ill individuals and Bilingual in Spanish is a plus.
