RN - Group Home Position

As a Registered Nurse with Assured Care, you will play a vital role in providing high-quality care to seniors, vulnerable adults, and individuals with disabilities and/or chronic illness in our group home setting in Minneapolis, Minnesota. Responsibilities:

• Provide direct patient care, including administering medications, wound care, and other treatments as needed
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Monitor and report patient progress, identifying areas for improvement and making necessary adjustments to care plans
• Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care
• Educate patients and their families on disease management, self-care techniques, and community resources
• Maintain accurate and detailed records of patient care and treatment
• Participate in quality improvement initiatives and contribute to the development of policies and procedures

Requirements:

• Current RN licensure in the state of Minnesota
• Strong clinical skills and experience in gerontology or long-term care
• Ability to work effectively in a fast-paced environment and prioritize tasks efficiently
• Excellent communication and interpersonal skills
• Ability to maintain confidentiality and handle sensitive information
• Strong attention to detail and ability to maintain accurate records
