As a Home Health Speech Language Pathologist at CenterWell Home Health, reporting to the Clinical Manager, you will play a vital role in helping patients regain strength, mobility, and independence—all from the comfort of their homes. Flexible 24-32 hours per week. Responsibilities Evaluate, direct and provide speech/language pathology service to patients in the home or facility Participate in the development and periodic review of the Plan of Treatment and Plan of Care Use professional skills and judgment in assessing and treating disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize the effects of such disorders and conditions Administer and interpret diagnostic tests and applications of therapeutic treatments including audio logic screening Observe, record and report changes in the patient's condition and response to treatment to supervisor and/or physician Provide instruction and training to patients in use of alternative communication systems when appropriate Provide counsel and instruction to patients, families and healthcare staff Maintain and submit documentation as required by the company and/or facility, including written reports of evaluations, visits, summaries, care plans, care coordination activities and progress reports as required by company policy Participate in care coordination activities and discharge planning Maintain the highest standards of professional conduct concerning confidential information. Share information only when the recipient's right to access is clearly established and sharing is in the best interests of the patient Required Qualifications Minimum six months experience as a speech therapist/speech language pathologist Current and unrestricted license Current CPR certification A valid driver's license, auto insurance and reliable transportation Preferred Qualifications Home Health experience Pay Range $54.00 - $76.00 per visit/unit $85,400 - $117,500 per year base pay Benefits Humana, Inc. and its affiliated subsidiaries offer competitive benefits that support whole-person well-being. Additional Information TB Screening Program: This role is considered patient-facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB. Driver Safety Program: This role is part of Humana's driver safety program and requires a valid state driver's license and personal vehicle liability insurance. The individual must carry vehicle insurance in accordance with the residing state minimum required limits (e.g., $25,000 bodily injury per person, $25,000 bodily injury per event, $10,000 property damage or higher).
