Home Care Admissions Nurse
The Holyoke Visiting Nurse Association, Inc., established in 1905, is dedicated to providing quality skilled care to individuals in the comfort of their home. We coordinate and deliver comprehensive, cost-effective services, including nursing, physical therapy, occupational therapy, speech therapy, social work and home health care. In addition, we are committed to promoting community health through education and special programs.
The Holyoke Visiting Nurses Association, Inc. is looking to hire a full-time Home Care Admissions Nurse. Position Summary: The Home Care Admissions Nurse evaluates patients referred to home care services and determines the types of services to be provided. Conducts interviews of the patient and family to determine the appropriateness for services. Responsible for obtaining the initial and ongoing referral documentation from physicians, insurers and providers. Coordinates the development of home care plans specifying the frequency and duration of the services provided. The Home Care Admissions nurse will complete admission, referral and insurance forms.
Requirements: Education: Bachelors Nursing (BSN) preferred. Licensure/Certification: Current MA Registered Nurse (RN) license CPR Certified Experience: One (1) to three (3) years home care experience Demonstrated computer / EMR skills
