Transition Coordinator
National Church Residences
National Church Residences seeks a Transition Coordinator to facilitate moving individuals from long-term care facilities to home settings. This role works from home with frequent travel across the Western half of Ohio and supervises a team of coordinators to ensure policy adherence and successful transitions. Candidates should have a Bachelor’s in social work or a related field and at least 3 years in social services for the elderly or related populations, including experience with HOME Choice #J-18808-Ljbffr
