Home Care & Hospice Virtual Care Center Coordinator Registered Nurse (RN) Employment Type: Full time Shift: Description: Hiring for the following hours/shifts: Full-Time: Monday-Friday 9:30am-6:00pm EST w/every 3-weekend rotation & 3 holidays a year Full-Time: Monday-Friday 4:00pm-12:30am EST w/every 3-weekend rotation Part-Time: Saturday & Sunday 2:00pm-12:30am EST Contingent/PRN: 2:00pm-12:00 am (must be available to work at least 30 hours over 6 week schedule) Build a Career. Meaningful 1:1 Patient Care - Build real relationships and see the direct impact of your work every day. What You Will Do: Conduct virtual assessments andmonitorpatients' conditions remotely. Develop and coordinate individualized care plans in collaboration with the interdisciplinary team. Provide virtual education and support to patients, families, and caregiversregardingsymptom management and end-of-life care. Liaise with field nurses, physicians, social workers, and other team members to ensure comprehensive care delivery. Respond promptly to urgent virtual consults and triage patient needs effectively. Utilize telehealth technology to conduct regular check-ins and follow-ups with patients and families.
