Overview

DaVita Kidney Care brings industry-leading clinical quality to patients on-site at skilled nursing facilities (SNF) through DaVita Skilled Nursing Dialysis Services. The DSD team works to develop the infrastructure and rapidly grow the service line, collaborating with cross-functional resources across the organization. This role is ideal for teammates who have dialysis experience, thrive in a startup-like environment, and are motivated by improving patients’ lives. Teammates must be comfortable working autonomously and collaborating with others across the Village.

What you can expect

In this role, you take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track. PCTs will work in conjunction with dialysis RNs to provide dialysis care.

Responsibilities

• Provide comprehensive patient care in collaboration with the dialysis team.
• Assist with hemodialysis treatment setup, monitor patients, record vital stats, and document observations.
• Operate and troubleshoot dialysis equipment as directed.
• Support SNF partners and coordinate with cross-functional teams to meet patient needs.

Qualifications

• High school diploma or equivalent
• OCDT certification (MUST BE OCDT CERTIFIED)
• Basic computer skills; proficiency in MS Word and Outlook

• Training, teaching, or healthcare education background
