Responsibilities:

Provide direct patient care to residents, including administering medications, wound care, and treatments as ordered by physicians

Develop and implement individualized care plans in collaboration with interdisciplinary care teams

Conduct routine vital sign checks, monitor resident condition, and report changes to nursing staff and physicians

Participate in quality improvement initiatives to enhance resident outcomes and satisfaction

Communicate effectively with residents, families, and healthcare team members to ensure seamless care coordination
