- Conduct thorough evaluations of patients in both inpatient and outpatient contexts, crafting personalized treatment plans
- Implement targeted physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
- Collaborate with nurses, physicians, case managers, and other rehabilitation experts to ensure optimal patient outcomes
- Educate patients and their families regarding therapeutic goals, discharge planning, and home exercise routines
- Accurately document assessments, treatments, and patient progress in alignment with facility and regulatory requirements
- Engage in care team meetings and interdisciplinary conferences to promote cohesive care
- Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
- State licensure as a Physical Therapist
- CPR certification is highly preferred
- Additional specialized certifications (e.g., in geriatrics or neurology) are advantageous but not mandatory
- Preferably possess prior experience in a similar role
- Capable of addressing complex medical and mobility challenges
- Exhibit exceptional communication and teamwork abilities
- Proficient in digital charting and clinical documentation systems
