Description

• Conduct patient evaluations in both inpatient and outpatient settings, customizing treatment plans to individual needs

• Work collaboratively with nurses, physicians, case managers, and other rehabilitation specialists to optimize patient outcomes
• Provide education to patients and families on treatment objectives, discharge planning, and home exercise regimens
• Maintain comprehensive documentation of evaluations, treatments, and patient progress in compliance with facility and regulatory guidelines
• Engage in interdisciplinary care team meetings to ensure cohesive patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent accredited degree

Certifications

• Current state licensure as a Physical Therapist
• CPR certification strongly preferred
• Specialty certifications such as in geriatrics or neurological conditions are advantageous but not required
