Job Title: Licensed Practical Nurse 
Service Location: Cresco, PA; Tobyhanna, PA; Lake Ariel, Pa
Immediate Needs:
Cresco, PA - LPN need for TRACH - VENT - G/J Tube - Large Dog Friendly Home!
- Friday and Saturday 11pm to 7am (times can be flexed if needed) and Saturday and Sundays 7am to 3pm (times also flexible if needed)
CRESCO,PA - LPN/RN needed for Friday and Saturday 7 am to 11 pm ( times can be flexed if needed) There is a dog and a bird in the home.
Tobyhanna, Pa - LPN for g-tube care , PART-TIME
Tobyhanna, PA - LPN for trach/vent care , PART-TIME
Lake Ariel, Pa - LPN Weekends Saturday and Sunday 7am-11pm Shifts can be split has 18 hours a day on both days / Monday , Thursday and Fridays 6pm-6am

Responsibilities:

• One to One patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members
