SUMMARY: The case manager assumes primary responsibility for each member on an assigned caseload and coordinates the implementation of the program Service Plan and maintains each member’s record. Essential Functions Develop and ensure the implementation of the program Service Plan. Serve as liaison between the program and each member’s support system. Complete and submit all assessments as required. Maintain and update all service records. Maintain an expertise in the field through attendance at meetings, trainings, continuing education or other appropriate educational programs. Attend interdisciplinary team meetings as needed to participate in curriculum design. Qualifications High school diploma or equivalent At least one-year case management experience in a program for developmentally disabled adults, or three years’ experience in Human Services or a related field. Skill in both written and oral communication. Ability to working both independently and as a team member. Ability to effectively participate in the design, implementation, and coordination of services. Ability to complete and disseminate all required forms and documents. A qualifying criminal background check, driving record, and a valid driver’s license required. Why join our Team?
