Pediatric Clinical Pharmacist - Neurology

Overview Provides leadership in a specialized healthcare team and the patient to assure optimal cost-effective drug therapy. This includes developing specific pharmacotherapeutic goals, recommending a regimen and developing a monitoring plan to meet the individualized drug therapy needs of the patient. Key Responsibilities Lead efforts in prevention and reporting of medication errors and adverse drug reactions according to policy. Prescribe and dispense medication, provide drug information, and manage medication therapy in accordance with federal, state, and local regulations. Optimize pharmacotherapy through medication reconciliation, patient counseling, and prior authorization review. Assist with retail prescriptions and guide patients in navigating outpatient pharmacy services. Collaborate with pharmacy, nursing, and medical staff; communicate effectively with providers and patients. Qualifications Licensed pharmacist in Tennessee. Bachelor’s degree in pharmacy or related field. Minimum 1 year of relevant work experience; 2 years of pediatric hospital experience preferred. Completion of an ASHP accredited PGY‑1 residency program in a pediatric setting preferred. Strong communication skills and ability to work with multidisciplinary teams.
