Requirements
- Must be a Licensed Practical Nurse in the state or hold an unencumbered compact license.
- Graduation from an approved school of practical nursing.
- Experience in medical/surgical nursing and/or home health care.

Roles & Responsibilities
- Provide direct nursing care to patients under the direction of a Registered Nurse Case Manager.
- Make home visits to assist in the provision of care to meet patient and family needs.
- Communicate changes in patient condition and necessary updates to the plan of care to the appropriate team members.
- Assist in performing specialized duties related to end-of-life care.
- Observe, document, and report the patient's physical and emotional needs and reactions to care.
