As a Hospice Licensed Practical Nurse, you will play a crucial role in assessing patients' health statuses and delivering nursing interventions in collaboration with an interdisciplinary team. Your responsibilities will include:

• Making home visits under the supervision of an RN Case Manager to meet patient and family needs
• Communicating changes in patient conditions and necessary care plan adjustments to team members and caregivers
• Assisting RNs and medical providers with specialized end-of-life care duties
• Observing, documenting, and reporting patients' physical and emotional needs and reactions to care

Requirements for this role include:

• Must be a Licensed Practical Nurse in Vermont and New Hampshire or hold an unencumbered compact license
• Graduation from an approved school of practical nursing
• Experience in medical/surgical nursing and/or home health care
• Need 1 year experience in setting
