About the Role:

As a Registered Nurse with Family Support Care, you will be responsible for:

• Providing nursing care to patients under the supervision of a licensed physician

• Conducting patient assessments, developing individualized care plans, and implementing interventions as needed

• Collaborating with other healthcare professionals to ensure seamless care transitions

• Maintaining accurate and detailed records of patient care and treatment outcomes

• Participating in quality improvement initiatives to enhance patient outcomes and satisfaction

Responsibilities:

• Provide direct patient care, including administering medication, taking vital signs, and performing treatments

• Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams

• Identify and address potential safety concerns and risks, reporting any incidents to supervisor

• Maintain accurate and detailed records of patient care, treatment outcomes, and medication administration

• Participate in patient education and support, promoting health literacy and self-advocacy

Requirements:

• Current Registered Nurse (RN) license in the state of Ohio
