Occupational Therapist – PACE Program Pay Range: $38.52–$47.00 per hour, based on experience Sign-On Bonus: $10,000 Location: Maryland Employment Type: Full-Time About the Position We are seeking a compassionate and experienced Occupational Therapist (OT) to provide clinical occupational therapy evaluation and treatment to participants in a Program of All-Inclusive Care for the Elderly (PACE). This position focuses on helping older adults maintain independence, safety, and quality of life while living in the community. The Occupational Therapist will provide acute, restorative, and maintenance therapy based on individual participant needs. Services may be provided at a Day Center, nursing home, or in participants' homes. This is an excellent opportunity for an Occupational Therapist who enjoys working with older adults and collaborating with an interdisciplinary team to deliver individualized, participant-centered care. Key Responsibilities Conduct comprehensive occupational therapy evaluations and assessments. Develop individualized treatment plans and care plans based on participant needs and goals. Provide occupational therapy treatment consistent with established goals and clinical recommendations. Provide acute, restorative, and maintenance therapy as appropriate. Evaluate participant progress and adjust treatment plans as needed. Provide services in various settings, including the Day Center, nursing facilities, and participants' homes. Coordinate home modification programs to promote independence, accessibility, and safety. Train and educate participants, family members, caregivers, and staff on strategies to maximize safety and functional abilities. Accurately document evaluations, treatment sessions, progress, and other required clinical information. Collaborate with physicians, nurses, therapists, social workers, caregivers, and other members of the interdisciplinary care team. Mentor, coach, counsel, and provide guidance to department staff as appropriate. Maintain knowledge of applicable laws, regulations, organizational policies, and compliance requirements. Participate in required training, educational programs, meetings, and professional development activities. Fulfill designated responsibilities during disaster recovery and business continuity situations. Perform other duties as assigned. Required Qualifications Bachelor's degree in Occupational Therapy from an accredited program – Required. Current, active, and unencumbered Occupational Therapist license in Maryland – Required. Employee Medical Statement meeting applicable job requirements – Required. Preferred Qualifications 2+ years of experience as an Occupational Therapist – Highly Preferred. 1+ year of experience working with a frail or elderly population – Preferred. Appropriate clinical placement experience may be considered in lieu of some elderly-population experience. Experience working in a PACE, geriatric, rehabilitation, home health, or community-based healthcare environment is a plus. Participants are generally age 55 or older, live within a designated service area, meet applicable requirements for nursing-home-level care, and are able to safely remain in the community. The program provides comprehensive medical, healthcare, rehabilitation, social, and supportive services. Services may include adult day health programs, rehabilitation therapy, nutritious meals, social engagement, and care provided in participants' homes. Care is coordinated around each participant's individual needs and is delivered through an interdisciplinary team focused on promoting health, independence, safety, and quality of life.
