Full-time Therapy Care PharmacistSoleo Health is seeking a Full-time Therapy Care Pharmacist to support our Specialty Infusion Pharmacy in Centennial, CO. Join us in Simplifying Complex Care!The Position:Therapy Care Pharmacists utilize national and local resources to facilitate high quality care of patients with specialty pharmacy needs. They collect and assess clinical data from patients, implement and monitor care plans, communicate with prescribers to obtain orders, report outcomes, make therapeutic recommendations, and dispense prescription medications and supplies, and supervise support activities of therapy care specialists in accordance with all laws, regulations, and policies and procedures set forth by state and federal law, accrediting bodies, and Soleo Health.Responsibilities include:Collect clinical data for patients, caregivers, and nurses and assess data to monitor efficacy and safety of a prescribed specialty therapiesCollaborate with prescribers to implement or modify existing treatment plansAct as a subject matter expert and clinical knowledge resource to patients, prescribers, referral sources, and fellow team membersCoordinate activities with local or national pharmacy team members, including pharmacists, pharmacy technicians, nursing, intake/admissions, reimbursement, and sales staff to coordinate to ensure an efficient customer focused approach and optimal clinical patient outcomesDocumentation of all patient interaction and care coordination in the designated system of record for clear and consistent communicationDispense specialty medication and suppliesPromotes teamwork, proactivity, leadership, service, mentorship, positivity, and optimism to strengthen staff and contribute to excellent work environment and customer serviceEmbraces Soleo's Quality Assurance and Performance Improvement initiatives, and plays an active role in supporting these programsAssist in orientation and training of new pharmacy staffSupervises activities of therapy care specialists and other support personnelEnsures all pharmacy activities and documentation comply with state and federal law, accreditation body standards, Soleo Health policies and procedures, and payor, manufacturer, and referral source requirementsObtains additional state pharmacy licenses as requestedSchedule:Full-time, 40 hours per week, Monday to Friday, 8:30am - 5pmMust be able to participate in on-call schedule rotation (1 week approximately every 3-4 months)Must be licensed as a Pharmacist in the state of ColoradoRequirements:Prior experience in home infusion and/or specialty pharmacy preferredDoctor of Pharmacy (Pharm D) or R.Ph with relevant clinical experiencePost graduate residency and/or trainingApplicable active state licensureAdditional specialty certification or training may be requiredPrior experience with CPR+ pharmacy computer system preferredExcellent clinical and customer service skillsBasic to intermediate efficiency with Microsoft Excel, Word and Outlook.About Us:Soleo Health is an innovative national provider of complex specialty pharmacy and infusion services, administered in the home or at alternate sites of care.
