Ensures safe, appropriate, and economical use of drugs through the application of specialized skills and knowledge in pediatric patient care. Critical Care Clinical Pharmacist This position covers our intensive care units overnight. One pharmacist will cover PICU and ED, the other pharmacist will cover NICU and CICU. Based on our practice model, the pharmacist will be expected to respond to all emergencies within their assigned area. The Department of Pharmacy provides comprehensive clinical services to patients at Children's Hospital Colorado. We have organized clinical pharmacy services that complement the team approach to providing care. Our clinical pediatric pharmacists are available 24 hours a day and help ensure the safe and effective use of medication in children - as well as promote health, wellness and disease prevention. Our specially-trained pharmacists work with physicians and other healthcare professionals to determine the best medication regimen and monitoring plan for the pediatric patient. Clinical pharmacy services are integrated into virtually all inpatient care units as designated pharmacists round with the medical teams. Through this direct involvement, pharmacists are empowered to assist in a great deal of therapeutic decision making and therapeutic drug monitoring activities. Additional Information Hours per week: 36, eligible for benefits Shift: Nights, 2100 - 0800 (7 nights on, 7 nights off) Relocation assistance available for qualified external applicants. Duties & Responsibilities Provides drug information services by responding to questions regarding medication compatibilities, adverse drug events, regimens, dosing, interactions, storage and disposal, compounding, therapeutic uses of medications and pharmaceutical supplies in a timely manner; using a systematic approach to effectively searching, retrieving, and evaluating the literature and appropriately communicating and applying the information to the patient care situation; and assisting with the development, maintenance, and implementation of hospital and department policy and procedures related to medication use, storage, restrictions, and administration. Sustains and implements quality and safety initiatives and practices in the department and throughout the hospital. Reports adverse drug events and medication variances using the hospital variance reporting program (QSRS). Sustains clinical pharmacy services throughout the organization by providing pharmaceutical care for pediatric patients of all age ranges through participation in multi-disciplinary team rounds and/or care, emergency response, education, guidance, interpretation and analysis of medication related therapeutic issues; documenting pharmaceutical interventions according to departmental standards; performing therapeutic drug monitoring and disease outcome analysis; and timely verification of medication orders in an electronic health record system. Participate in general pharmacy operations throughout the organization through appropriate medication preparation, dispensing, and inventory control. Provides oversight to pharmacy technicians, interns, residents, and students as needed. Minimum Qualifications Education: Doctor of Pharmacy or Bachelor of Science in Pharmacy degree Experience: Completion of one year pharmacy residency Preference will be given to those applicants who have at least one year of pediatric pharmacy experience and/or have completed a residency in pediatrics. Equivalency: One year of pharmacy work experience accepted in lieu of residency Certification(s): BLS certification (American Heart Association, American Red Cross, or American Safety and Health Institute accepted) Licensure(s): State of Colorado Department of Regulatory Agencies (DORA) Pharmacist license Salary Information Pay is dependent on applicant's relevant experience.
