Location: Fremont, CA area
Facility Type: Post-Acute / Skilled Nursing
Schedule: Monday–Friday, 35 hours per week
No Weekends
Pay: $54–$59 per hour

Talented Medical Solutions is seeking a compassionate and skilled Occupational Therapist to join a supportive post-acute care team in the Fremont, CA area . This is a full-time permanent opportunity offering a steady Monday–Friday schedule with no weekends .

The Occupational Therapist will evaluate patients, develop individualized treatment plans, and provide therapy services that help patients improve independence, daily living skills, strength, coordination, and overall functional ability.

With a 199-bed care environment , this position offers the opportunity to support a wide range of patient needs while working closely with a collaborative interdisciplinary team. The therapy team plays an important role in helping patients rebuild confidence, regain independence, and improve quality of life after illness, injury, or surgery.

• Evaluate patients and develop individualized occupational therapy treatment plans

• Provide skilled therapy services to improve daily living skills, strength, mobility, coordination, balance, and independence

• Monitor patient progress and adjust treatment plans as needed

• Collaborate with nursing, therapy, and care teams to support patient goals

• Educate patients, families, and caregivers on therapy goals, safety, and functional strategies

• Complete documentation accurately and timely

• Follow all facility, state, and federal guidelines

• Graduate of an accredited Occupational Therapy program

• Skilled nursing, post-acute, rehab, long-term care, or geriatric experience preferred

• Strong clinical judgment and documentation skills

• Excellent communication and teamwork abilities

• Compassionate, dependable, and patient-focused

• Active California Occupational Therapist license required

• NBCOT certification required

• CPR/BLS certification required

• Must be eligible to work in a skilled nursing/post-acute care setting

• $54–$59 per hour

• 35 hours per week

• Monday–Friday

• No weekends

• Permanent full-time position
