What You’ll Do

Perform comprehensive nursing assessments and develop individualized plans of care

Provide skilled nursing care, patient education, and care coordination in the home

Collaborate with physicians, therapists, medical social workers, and other members of the care team

Monitor patient progress and communicate changes in condition promptly

Complete accurate and timely documentation while maintaining Medicare and agency standards

Qualifications

Current Florida Registered Nurse (RN) license

Valid driver’s license and reliable transportation

Excellent clinical, communication, and organizational skills

Home health experience required

OASIS experience required
