BETHESDA HOME HEALTH AND HOSPICE
- Reports to: Hospice Director
- Supervisory Responsibilities: Hospice Care Aides
- Internal Relationships: Other RN Case Managers, Scheduling Coordinator, Bethesda Social Service departments, Volunteer Coordinator, Housing Managers and Coordinators, Contracted Therapists and Medical Director.
- External Contacts: Medical Director, Physicians, Pharmacists, Health Service personnel in surrounding area, Kandiyohi County Family Services Social Workers and Public Health Nurses, Nursing Home Social Workers, Hospital Discharge Planners, Insurance Company personnel, Patients and Family members, and other community providers.
Essential Requirements of Work: (Minimum qualifications necessary to function at full productivity.)
- Education:
- Graduate of an accredited school of nursing.
- Current Minnesota registration as a Registered Nurse. Must maintain licensure.
- Current CPR Certification or ability to immediately obtain certification
- Experience:
- Strong proficiency in nursing skills.
- Hospice experience preferred.
- Previous management experience preferred.
- Basic Computer skills.
- Personal:
- Highly developed communication skills (oral and written).
- Proven time management skills.
- Understanding of basic quality principles.
- Adaptable to various work settings.
- Self-motivated.
- Attention to detail.

- Own vehicle, current valid driver's license, and proof of current automobile insurance.
- In accordance with CMS Vaccine Mandate, must provide proof of Covid-19 Vaccine or submit documentation for exemption
- Job Knowledge:
- Knowledge of nursing policies and procedures.

- Knowledge of State Federal regulatory requirements.
- Knowledge of the Nurse Practice Act.
- Ability to assess patient's condition with knowledge to provide necessary action.

Frequent exposure to infection. Principle Job Responsibilities: Responsibility: Provide and monitor quality care. Tasks:
- Follow State and Federal regulations and agency policies and procedures.
- Provide coordination of patient care to ensure continuity and quality care.
- Deliver skilled services efficiently and effectively to meet patient's and family's/caregiver's needs.
- Coordinate and review hospice referrals with IDT team.
- Complete an initial, comprehensive, and ongoing assessment of patient and family/caregiver to determine hospice needs. Provide a complete physical assessment and history of current and previous illness.
- Collaborate with physicians, other health care professionals, patients, and families/caregivers in developing a comprehensive coordinated plan of care, which includes objective goals for patient improvement in health and functional status. Evaluate plan of care on continuous basis, update physician and other therapies of changes in patient status and need for revision of plan of care. Include the patient and family/caregivers in the planning process.
- Teach the patient and family/caregiver self-care techniques as appropriate.
- Provide medication, diet and other instructions as ordered by the physician and recognize and utilize opportunities for health counseling with patients and families/caregivers.
- Ensure that physician orders are communicated to the agency's staff with accuracy and thoroughness and understanding.
- Ensure that arrangements for equipment and other necessary items and services are available.
- Provide and maintain a safe environment for the patient, family/caregiver, and Hospice Care Aides and other employees or volunteers.
- Initiate case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.
- Work in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver.
- Collaborate with Director and Human resources to provide strong orientation and training of team members.
- Assist with the scheduling of patients to Hospice Care Aides.
- Review documentation of Hospice Care Aides.
- Supervise Hospice Care Aides and complete supervisory visit documentation as required. Give feedback to the Hospice Care Aide as needed.
- Follow infection control guidelines to prevent the spread of disease and infection.
- Provide assistance to Hospice Care Aides in new or difficult situations.
- Select appropriate patient for Telemonitoring. Use and monitor results from Telemonitoring system.
- Assist with development, implementation, review, and revising of policies and procedure that guide and support the provision of services.
- Assist with development, implementation, review and revising of a Quality Assurance/Improvement Program.
- Procedures and treatments include but not limited to:
- Medication set-up and administration.
- Dressing changes.
- Catheterizations and catheter care.
- Colostomy care.
- Tube feedings.
- IV administration and site care.
- Vital Signs.
- Respiratory therapy including nebulizer treatments.
- Blood glucose monitoring.
- Blood draws.
- Complete, maintain and submit accurate and relevant clinical notes regarding patient's condition, pain/symptom management, and care given.
- Complete necessary paperwork for admissions, transfers, deaths, and discharges.
- Complete all required assessments.
- Document each visit in medical record.
- Complete Comprehensive and OASIS assessment when required.
- Complete Hospice Care Aide Care Plans prior to the initiation of those services.
- Complete Communication Log after communications with patients, family, health care providers, or staff.
- Carry on-call cell phone at all times when on-call.
- Respond to calls received, assess the situation, and provide direction.
- Document all phone calls.
- Perform Skilled Nursing Visits as needed.
- Participates in evening, weekend, and holiday rotation as needed/scheduled.
- Present self in a professional manner.
- Take referrals as needed.
- As opportunities arise, promote agency services to discharge planners, physicians, potential patients/families, and the general public.
- Report for and leave the work area on a timely basis.
- Participate in annual performance evaluation process.
- Write yearly personal goals.
- Participate in all required in-service programs at Bethesda.
- Maintain a professional conduct and pleasant, friendly attitude towards staff, residents, and visitors.
- Be willing to learn and change to meet needs that arise.
