This position is located in our Oakland City Center location, with expectations of home visits in the Bay Area. About Your Work The Community Health Advocate will assist members to locate and connect to needed services and resources within the medical system and community, helping to manage health more effectively. You will partner with medical systems and community resources, help members problem-solve barriers to health conditions, and navigate existing services. You will identify resource gaps and support implementation of new services to close those gaps. You will focus on Assesses psychosocial and social determinants of health needs for low-risk members and documents assessment results or augments available information in appropriate systems Consults with or refers members to licensed staff (social worker, nurse case manager, etc.) as required based on member social, health risk, and medical complexity Establishes relationships and partnerships with community resources, health plans, and providers by participating in community engagement activities with local agencies (e.g., faith-based organizations, community centers, government agencies, parks, recreation centers, and schools) Assists members with problem-solving barriers to health conditions by identifying, locating, connecting to, and navigating needed services, including home visits and helping with forms to access services Engages, builds rapport, and communicates with members and their support systems Identifies social determinants of health concerns, develops and documents a plan to address disparities in accessing care and services Documents member updates and progress notes in appropriate systems and submits timely reports Participates in Interdisciplinary Team Meetings and reports updates on member needs and progress Files and follows up on mandated reporting (e.g., Adult Protective Services, Child Protective Services) Other duties as assigned, including occasional home visits as directed by the Manager The Skills, Experience & Education You Bring Communication skills and ability to work with various members of the clinical team Electronic Health Record knowledge and ability to document following standards, policy, and procedures Time management and ability to manage a caseload with minimal supervision Medical assistant experience is a plus You Share Our Mission & Values You are passionate about improving the healthcare experience and want to be part of the Altais mission You are bold and curious—willing to take risks, try new things, and be creative You take pride in your work and are accountable for the quality of everything you do You are compassionate and demonstrate emotional intelligence, consider others in decisions, and strive to do the right thing You co-create, working well with others and valuing diverse thought and perspective You build trust with colleagues and customers by valuing honesty and transparency Altais values the contribution each team member brings to our organization. Roles involve a part to full-time schedule in an office setting with occasional home-based work. Activity level is sedentary with most of the workday at a desk. External hires must pass a background check and drug screen.
