POSITION SUMMARY
The Family Care Coordinator must have a Bachelor’s degree in Social Work (or other related field) with ability to provide in-home services to children and families and experience working with complex and multi-need families.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Other duties may be assigned according to qualification of staff and development of programs.
1. Direct Service
- Complete face-to-face intake with family within 24 hours to assess safety of the child(ren), family’s acceptance of services, and arrange for ongoing communication with the referring DCBS worker.
- Develop and maintain a Safety Plan, ensuring a copy is given to the family, and updating as needed.
- Act as the point of contact for the family and DCBS throughout the intervention.
- Assist with data gathering and other tasks associated with the family assessment, as needed.
- Ensure DCBS receives a copy of all documentation within contractual timeframes.
- Collaborate with the team and family to develop Family Agreements and Action Plans to meet the identified needs of the family.
- Facilitate implementation of the Action Plan by establishing contact with all providers identified on the plan, connecting providers with the family, maintaining ongoing communication with family and treatment providers, and coordinating care for the duration of services.
- Schedule and conduct team meetings to engage members of the team, family, community partners, and natural supports in review of progress toward identified goals.
- Utilize agency models to provide direct services to families referred for services targeted at reducing risks, improving child well-being, and preserving child placement with the family, as identified in the family’s Action Plan.
- Maintain face-to-face contact with each family as per program guidelines regarding frequency, intensity, and duration.
- Administer the Columbia Suicide Severity Rating Scale (C-SSRS) to family members according to agency policy to assess risk for suicide and take appropriate steps to mitigate identified risks.
- Connect family with community partners and resources to meet financial, environmental, and personal health needs, including the use of flex funds as needed, while assisting the family with developing natural support systems.
- Maintain communication with DCBS worker regarding family’s participation, safety risks, identified needs, and progress in treatment.
- Facilitate case closure meeting to determine family’s readiness for discharge. Develop an Aftercare Plan jointly with family, team, and community partners and assist in making referrals for ongoing services as needed.
- Participate in weekly case consultation to review families’ engagement in treatment and progress toward service goals.
- Complete 3, 6, and 12 month aftercare contacts and document in ChartOnline within agency and contractual timeframes
- Regularly travel throughout the region in order to participate in home visits, make partner contacts, and as needed to assist families with meeting their goals. Occasionally travel out of region for trainings and other meetings.
- Maintain flexible schedule based on needs and availability of clients. Work time may include evenings and weekends.
- Participate in on-call rotation to facilitate availability of a supervisor back-up to specialists 24 hours/day, 7 days/week.
2. Continuing Education & Professional Development
- Create an Individual Learning Plan with Supervisor to identify required and needed areas of training in addition to areas of interest to enhance service delivery to families receiving services.
- Attend training in the Solution Based Casework model within 6 months of hire or as soon as the training is offered by the state.
- Complete the Solution Based Casework certification process.
- Complete training in the North Carolina Family Assessment Scale within 2 months of hire.
- Attend trainings in fidelity measures necessary to complete job functions.
- Attend meetings, seminars, and other educational functions of the agency as identified by supervisor.
- Obtain 40 CEUs per year, including 20 by June 30.
- Complete assigned Relias Learning courses by given deadlines.
- Complete all CEUs required to maintain professional license, as applicable.
3. Record Keeping & Documentation
- Maintain and submit needed records in ChartOnline (electronic records system) for the purpose of quality assurance, documentation of service delivery, and agency billing.
- Complete program-specific case documentation within agency and contractual timeframes.
- Collaborative documentation will be utilized to complete service tickets, ChartOnline and other documentation and forms while in session with clients and/or guardians to ensure client involvement with treatment.
- Complete Service Tickets and other documentation in ChartOnline in accordance with program deadlines.
- Upload case-related documentation to ChartOnline in accordance with agency requirements.
- Provide progress reports for each current case to DCBS personnel in accordance with program guidelines and as requested by DCBS.
- Submit receipts weekly and in accordance with submission policy.
- Enter case information into the In-Home Services Database, as needed.
4. Special Projects
- Assist with any special duties or responsibilities assigned by agency director or another member of management.
5. Professional Performance and Conduct:
- Meet expectations as defined in the Performance Evaluation and Competency Checklists.
- Dress professionally as outlined in the KVC Employee Manual.
- Maintain confidentiality of client and agency information. All KVC employees are privy to confidential client information. Due to complex treatment needs of our clients, service providers have contacts that require written and verbal information exchange with different professionals and agencies. A strict code of behavior must be maintained to protect the special nature of divulged information.
- Follow reporting laws. When a child is endangered, in areas of health, neglect, physical and/or sexual abuse, all staff are legally obligated to report that information to authorities.
- Maintain client rights as outlined in agency manuals.
- Follow risk management policies as outlined in agency manuals.
- Follow KVC emergency and safety procedures as outlined in agency manuals.
- Maintain a current Driver’s License and automobile insurance (as required by agency’s insurer).
- Maintain professional behavior and attitude when interacting with management, colleagues, clients, and community partners in any setting.
- Follow code of ethics of one’s profession (e.g., Social Work, Psychology, Counselors, Marriage & Family Therapists).
