Saint Peter's Healthcare System is seeking a Case Manager RN to join our magnet-designated Care Coordination team. You will perform discharge planning evaluations, assess patient needs, and collaborate with interdisciplinary rounds to optimize care and reduce readmissions. Requirements include a NJ RN license, BSN preferred or obtained within 3 years, 3–5 years in acute care, strong critical thinking and communication, and weekend/holiday availability. #J-18808-Ljbffr
