Full-Time River Bend Nursing and Rehab is seeking a dedicated and compassionate Social Services Director to join our team. This full-time position plays an important role in supporting the social and emotional well-being of our residents while ensuring the effective operation of the Social Services Department.

Full-Time River Bend Nursing and Rehab is seeking a dedicated and compassionate Social Services Director to join our team. This full-time position plays an important role in supporting the social and emotional well-being of our residents while ensuring the effective operation of the Social Services Department.
Key Responsibilities
- Develop comprehensive social assessments and care plans addressing residents' social and emotional needs, with actionable and realistic goals
- Participate in pre-admission and admission processes to help ensure smooth transitions for residents and families
- Provide compassionate support to residents and families during periods of adjustment, crisis, or special circumstances
- Maintain accurate documentation of residents' behavioral and psychosocial status
- Facilitate individual and group counseling sessions based on residents' needs
- Establish and develop volunteer programs to enhance social services initiatives
- Orient new residents and their families to the facility and available services
- Collaborate with the interdisciplinary team to support comprehensive resident care
- Bachelor's Degree in Social Work or a related field from an accredited institution
- Knowledge of social services practices, regulations, and guidelines applicable to long-term care
- Excellent interpersonal, communication, and organizational skills
- Ability to work independently while maintaining professional standards and compassionate care
- Previous long-term care experience preferred
