One Community. One Mission. One California. Responsibilities Perform social work assessments and interventions for inpatients, outpatients, and emergency department patients. Provide patient‑ and family‑centered care that values self‑determination and compassionate ethical problem‑solving. Perform comprehensive assessments and develop treatment plans that integrate medical, social, and resource issues. Collaborate with all healthcare team members to develop and implement patient‑centered plan of care. Qualifications At least two years of clinical or medical social work experience. Master’s degree from a school of social work accredited by the Council of Social Work Education. Licensed Clinical Social Worker in California. Excellent customer service and presentation skills. Strong interpersonal and written communication skills. Strong analytical and problem‑solving abilities. Ability to manage multiple tasks or projects effectively. Ability to work independently with high degree of attention to detail. Ability to work efficiently in a fast‑paced environment with changing priorities.
