Candidates must demonstrate excellent verbal and written communication skills, as this role requires frequent coordination with outside facilities and physicians regarding patient care and information. Candidate must have all scheduling restrictions/limitations resolved prior to start (ie. childcare, transportation, etc). This role is full time 40hr/wk, and the clinic needs someone who can show up on time and as scheduled to provide excellent patient care.

Facility Highlights: About: PacMed 65+ Health Center is a primary care model (clinic within a clinic) serving our elderly patients (age 62 and up), we are starting with 2 clinicians, and will need 2 MAs (standard MA workflow, working to the top of their scope) to help support the program. Value-based care, team model, goal is to minimize hospital admits/emergency department utilization for our complex patients.
