- Average Caseload: 6 Visits Per Day

Responsibilities
- Complete Start of Care (SOC), evaluations, reassessments, and discharges
- Develop and manage patient plans of care
- Supervise PTA staff
- Manage and schedule your own patient caseload
- Provide home health therapy services throughout York and Eastern Adams County
Requirements
- Active Physical Therapist license
- Graduate of an accredited PT program
- CPR (if required)
- 1+ year of Home Health experience highly preferred
- Home Care Home Base (HCHB) experience highly preferred
- OASIS experience preferred
