Schedule : Exempt (salary), min 40 hours per week | Monday - Friday | 8am - 4:30pm | No holidays! No weekends!

What You’ll Do:

As an experienced Nurse Practitioner with Thome PACE, you’ll be part of an interdisciplinary team caring for medically complex older adults. You’ll collaborate closely with nursing, social work, therapy, pharmacy, dietetics, home care, transportation, and scheduling to design the right care plan for each participant. The Nurse Practitioner supports clinically appropriate, participant centered, and resource responsible care.

Key Responsibilities

• Perform health histories, physical exams, and patient education.
• Order, interpret, and follow-up on lab tests and results.
• Perform selected procedures and document care both orally and in writing.
• Assess, diagnose, treat, and coordinate participant care. 
• Recognize abnormalities that require immediate referral for further intervention.
• Initiate and evaluate orders for medications, diagnostic studies, and treatments within physician-approved protocols.
• Provide emergency care as needed.
• Collaborate with ER physicians and hospital staff to manage care, prevent unnecessary hospitalizations, and facilitate expeditious discharge.
• Maintains clinical responsibility for an assigned participant panel, including routine and acute care, required assessments, meditation management, diagnostic testing, specialty follow up, and ongoing clinical monitoring. 
• Ensures orders, referrals, test results, specialty recommendations, hospitalizations, emergency department visits, and significant changes in condition receive timely clinical review and appropriate follow up.
• Provides or coordinates clinical management for participants receiving palliative or end of life care, in collaboration with the IDT, participant, family, and external care partners.
• Act as a liaison with physicians, nurses, and other healthcare professionals to ensure integrated care.
• Collaborate with attending physicians to request consultations from contracted services/providers as needed.
• Coordinate care with outside providers, including nursing homes, assisted living, adult foster care, and hospitals.
• Participate in interdisciplinary team (IDT) meetings, contributing to holistic patient care planning.
• Actively participates as a member of the interdisciplinary team and collaborates in the development, implementation, monitoring, and evaluation of participant plans of care.
• Responds timely to IDT, nursing, scheduling, pharmacy, and other clinical communications requiring provider review or direction.
• Reviews missed, cancelled, or incomplete specialty services and provides timely clinical direction regarding continued clinical need, alternative treatment, or appropriate follow up.
• Collaborates with the IDT when services outside the established PACE network or service delivery system are being considered.
• Educate participants and families on preventive health maintenance, particularly for chronic conditions.
• Actively participate in family conferences to provide education and care planning.
• Actively engage in quality improvement (QI) processes to enhance care delivery.
• Completes clinical documentation, orders, assessments, referrals, follow up, and other assigned provider responsibilities.

Skills, Knowledge and Expertise

• Bachelor’s Degree in Nursing from an accredited college or university, with graduation from a certificate or accredited Nurse Practitioner program.
• A valid Nursing License and educational degree/diploma.
• Minimum of five years of combine experience as a Registered Nurse (RN) and Nurse Practitioner (NP). 
• Current Michigan Controlled Substance License and DEA registration. 
• Current specialty certification to practice as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP).
• Valid driver's license and reliable transportation
