Home Health RN Case Manager
PVH (Tommy Hilfiger/Calvin Klein)
PVH Corp is seeking a Registered Nurse (RN) Case Manager to deliver home health services in patients' homes in compliance with physician orders and plans of care. This role blends direct clinical nursing with intermittent case management to ensure high-quality, individualized care in the Pueblo, CO area. The RN will perform initial evaluations, coordinate care among patients, families, and providers, and ensure accurate documentation (OASIS/PAR) within required timeframes, while adhering to #J-18808-Ljbffr
