Conduct assessments and develop individualized service plans. Monitor client progress and document outcomes. Coordinate services with healthcare providers, families, and support staff. Ensure compliance with state and federal regulations. Advocate for individuals' rights, goals, and community inclusion. Participate in interdisciplinary team meetings and case reviews.
