For more information, please visit We encourage you to apply to become a part of our team.Description of WorkThe Primary Care Capitation Population Health Program Support Specialist will play a central role in the design, implementation and oversight of the Medicaid Primary Care Capitation Pilot under the Rural Health Transformation Program. The Program Support Specialist will work closely with the RHTP internal teams, Regional Hubs, and participating practices.
