Pediatric CoCM Behavioral Health Care Manager

Tonawanda Pediatrics is hiring a full-time CoCM Behavioral Health Care Manager to join the collaborative care team with the patient’s medical provider and psychiatric consultant. You will support and coordinate mental and physical health care for an assigned caseload, ensuring engagement and timely follow-up. You will document outcomes in the EHR, participate in weekly caseload consultations, and facilitate referrals for psychosocial services as clinically indicated.
