Community Healthcare Network is seeking a Full-Time Care Manager/ Depression to join the Behavioral Health Integrated Care team. The role focuses on delivering brief evidence-based interventions for depression and coordinating comprehensive care for patients. Responsibilities include crisis intervention, psychoeducation, PHQ-9 progress monitoring, and collaborative psychiatric discussions with PCPs.
