Community-Based Case Manager - Housing & Behavioral Health
The Bridge
The Bridge Bronx, NY is seeking Scattered Site Case Managers to support formerly homeless adults with serious mental illness and/or substance use disorders. Clients live in apartments and receive community-based services, including assessments and Support Plans in electronic records. You will link clients to medical and mental health care, coordinate with providers, facilitate groups, document timely care, and respond to crises using motivational interviewing and culturally competent approaches. #J-18808-Ljbffr The Bridge
