Floyd Healthcare Management, Inc., part of Atrium Health Floyd, is seeking a Registered Nurse Hospice Case Manager to serve patients and families in Floyd County. In this field-based role, you will assess patients, develop and update individualized plans of care, manage symptoms, provide nursing interventions, and educate and support caregivers. You will coordinate closely with physicians and an interdisciplinary team, monitor changes in patient condition, and maintain timely, accurate clinical documentation. The ideal candidate is a licensed RN with sound assessment and communication skills and a compassionate, patient-focused approach to end-of-life care.
