The Care Team Hospice seeks a driven Community Care Liaison to grow our presence in the community. You will build relationships with physicians, hospitals, and referral sources to increase awareness and utilization of our hospice services. You will educate providers, patients, and families on hospice value and guide them toward compassionate, high-quality care while meeting census targets through strong partnerships. #J-18808-Ljbffr
