Chosen Village Home Healthcare is currently hiring licensed Physical Therapists (PTs) and Occupational Therapists (OTs) to provide in-home therapy services throughout the Houston, Dallas/Fort Worth, and Austin areas.
This is a great opportunity for clinicians who value flexibility, autonomy, and making a real impact in underserved communities. ? Responsibilities:
- Travel to patients’ homes to provide skilled therapy services • Conduct evaluations and develop individualized care plans • Provide ongoing treatment and caregiver education • Coordinate with patients’ physicians and internal case managers • Complete timely clinical documentation in our EMR (HHAeXchange)
Qualifications:
- Current Texas PT or OT license (required)
- Minimum of 1 year clinical experience (home health preferred) • CPR Certification
- Reliable transportation & valid driver’s license
- Strong communication and organizational skills
