Clinical Social Worker (Nurse)

Essential Duties: Must be an LPN or RN! Direct, establish & plan the overall policies/goals for the Social Services Department Assist in the development, administering, and coordinating of department policies and procedures. Stay up to date on current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Administrator. Review department policies and procedures at least annually, and participate in making recommended changes When necessary, create required procedures Responsible for assessments, care planning & helping residents adjust Ensure appropriate discharge planning Ensure all residents have advanced directives in place Complete Admissions, Psychosocial Assessments and all other necessary assessments Occasional Community Outreach Education, Experience and/or Training: LMSW preferred LPN/RN license a plus Bi-lingual fluency in English and Spanish preferred Long-term Care experience required 2-4 years management experience. Comprehensive knowledge of City, State and Federal regulations pertaining to licensed skilled nursing facilities.
