Hospice Case Manager

MJHS Hospice is seeking an experienced Inpatient Hospice Clinical Coordinator to strengthen hospital partnerships, assess referrals for hospice enrollment, and provide General Inpatient (GIP) level of care with compassionate clinical skills. The RN will lead an interdisciplinary team, develop care plans, coordinate transitions, educate referrers and families, and ensure accurate documentation in accordance with laws and agency policies. #J-18808-Ljbffr
