Maternal Health Case Manager (SUD/CADC)

CarePlus NJ, Inc. seeks a dedicated Case Manager for our Maternal Wrap-Around Program (M-WRAP). You will support women who are pregnant or postpartum with substance use disorders, delivering intensive case management and linking families to housing, health, and employment resources. You will coordinate care with multiple providers, document progress, and advocate for clients while addressing child welfare considerations and neonatal concerns.
