Hospice RN Case Manager - Meaningful, Patient-Centered Care

LifeSource Hospice is seeking an experienced RN Hospice Case Manager to join our interdisciplinary team serving patients in Bergen County, NJ. You will manage a caseload of hospice patients, coordinate care across disciplines, and ensure compliance with CHAP standards and regulations. You will perform nursing assessments, provide skilled visits, and collaborate with physicians, social workers, and other IDT members while traveling to patient homes and facilities within the county. #J-18808-Ljbffr Lifesource-Hospice
