Family Nurse Practitioner FNP

Total Care Family Practice

Job Description

The Family Nurse Practitioner (FNP) will provide comprehensive healthcare services primarily to geriatric patients. This role involves assessing, diagnosing, and developing treatment plans tailored to the unique needs of older adults. The FNP will also educate and counsel patients and collaborate closely with physicians to ensure high-quality, holistic care.

Responsibilities

Conduct patient assessments focused on geriatric health
Diagnose and develop individualized treatment plans
Provide health education tailored to older adult patients
Offer patient counseling to support wellness and chronic disease management
Collaborate with physicians and healthcare teams for integrated care delivery

Preferred Qualifications

Minimum 1 year of experience in family nursing
Master's Degree in Nursing
Certification as a Family Nurse Practitioner (FNP)
