Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: 260 days Shift: 5x5 Days Hours per Shift: Not specified Experience: Two (2) years of experience in a home health setting within the last fifteen (15) years License: Current RN Licensure in Client Certifications: BLS required before the start date Must-Have: Recent experience conducting comprehensive assessments of patients Ability to work with technology in the workplace Excellent organizational, interpersonal, written, and verbal communication skills Ability to communicate and work effectively with multi-generational consumers Ability to perform comfortably in a fast-paced, deadline-oriented work environment Strong ability to work independently Description: This position involves conducting initial in-home pre-evaluation of patients. The evaluation includes assessing health status, strengths, care needs, and preferences. You will guide the development of individualized long-term care service plans. Review consumer medical documentation and health referral forms as relevant to each case. Enter evaluation data into electronic forms and transmit as required. Document any concerns, conflicting information, or other issues that arise during evaluations. Emphasize continuity of care to reduce fragmentation, duplication, and gaps in treatment plans. Discuss healthcare options, needed supports, service vendor options, and waiver options with consumers. Initiate communication between the Call Center and eligible consumers for enrollment. Enhance communication and collaborative relationships with interdisciplinary care team members. Collect quality review data and documentation to support outcome measurements. Record case notes into the CRM system. Identify opportunities for health promotion and illness prevention. Maintain a comprehensive working knowledge of community resources and services for the target population. Perform other duties as assigned by the Regional Nurse Manager or project management team.
