Required experience includes two (2) years in a home setting within the last ten (10) years. Candidates must have experience working for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care. Experience conducting assessments similar to UAS such as OASIS is required. Providing primary care to homebound individuals is also required. Care management for those receiving home and community-based services is essential. Nurses should have experience as a paraprofessional or Licensed Practical Nurse, caring for a minimum of two patients per week. Preferred qualifications include experience conducting assessments in UAS-NY, particularly in the last 365 days. Experience caring for a geriatric population and managing chronic diseases in the home setting is preferred. A combination of home care experience as a paraprofessional or LPN, along with RN experience caring for a minimum of two patients per week, is desirable. Examples of experience that may count toward preferred qualifications include home care experience with only one patient or case management limited to utilization reviews. Assessments conducted entirely telephonically do not satisfy required qualifications. Candidates must be prepared to conduct hybrid tele and in-field assessments. Individuals must sign the required form before starting. Job Responsibilities: Perform initial in-home pre-evaluations of patients, including health status, strengths, care needs, and preferences. Review consumer medical documentation and health referral forms as relevant to the case. Enter evaluation data into an electronic evaluation form and transmit as required. Document any concerns or conflicting information that arise during the evaluation process. Emphasize continuity of care to reduce gaps in treatment plans. Discuss health care options and supports with consumers. Initiate communication between the Call Center and eligible consumers for enrollment. Enhance communication with interdisciplinary care team members to facilitate service delivery. Collect quality review data and required documentation for outcome measurements. Identify opportunities for health promotion and illness prevention. Maintain a comprehensive working knowledge of community resources and network services for the target population. Meet all performance standards established for this position. Perform other duties as assigned by the Regional Nurse Manager or project management.
