Home Health RN

The Home Health RN is responsible for providing individualized goal-oriented nursing care. Care is provided in accordance with a physician's order. The position also serves as Case Manager in cases where this position is the lead discipline. Home health experience is required. Experience as a home care traveler with proficiency in OASIS is preferred. This role includes case management responsibilities. Homecare Home Base experience is preferred.
